# Codebook: FDA reports on home-use and manual beds and portable bed rails (product codes LLI, FNJ, FNK and QTC) Written before any report in the set was coded. Mobility Atlas study, October 2026. ## Pool FDA MAUDE device reports filed under product code LLI ("Bed, Therapeutic, AC-Powered, Adjustable Home-Use", 21 CFR 880.5100), FNJ ("Bed, Manual", 21 CFR 880.5120), FNK ("Bed, Hydraulic, Adjustable Hospital", 21 CFR 880.5110) or QTC ("Adult Portable Bed Rails For Medical Purposes", 21 CFR 890.5050), read in openFDA on 16 October 2026, that carry text: (a) every such report received 1 January 2019 to 31 December 2025 (423); (b) every such report received in 2026 from a maker other than Medline (20); (c) a systematic sample of 250 of the 2,078 reports with text that Medline filed in 2026 (50 consecutive reports at five evenly spaced positions of the openFDA result list). Pool: 693 reports. ## What you record for each report One line per report: `report_key,device,category,injury,fall,setting,finding,note` ### DEVICE (one letter), from the report text; the brand line may be used when the text does not say - B: an ADJUSTABLE BED (electric, semi-electric, manual crank or hydraulic) used as a home or care bed, including its built-in side rails. - R: a PORTABLE or add-on BED RAIL, bed handle, assist bar or safety ring sold separately from the bed. - O: something else (a consumer adjustable bed base, a mattress alone), or the report does not say. ### CATEGORY (one letter): what the report says went wrong. Precedence: the FIRST letter that applies wins. 1. E: ENTRAPMENT: a person's head, neck, chest, arm or leg was caught, trapped or wedged in or between a rail, the bed frame, the mattress, the headboard or footboard, or a gap. 2. H: FIRE OR HEAT: smoke, sparks, melting, burning smell, overheating of the pendant, cable, motor or control box. 3. M: UNEXPECTED MOVEMENT: the bed moved, raised, lowered or tilted on its own, did not stop, or a section dropped suddenly. 4. R: RAIL: a side rail, bed rail, assist bar or handle broke, came loose, would not latch, released or fell, with no entrapment. 5. F: FRAME: the frame, a weld, a deck, a section, a leg or a caster broke, cracked, bent or collapsed, or the bed tipped. 6. C: CONTROLS OR MOTOR: the pendant, hand control, motor, actuator, cable, power supply or electronics failed, so the bed or a section would not move (no smoke or sparks). 7. K: BRAKES OR CASTERS: the bed would not stay still, brakes did not hold, a caster lock failed. 8. L: FALL from or out of bed with no failure from 1-7 named. 9. X: no device problem stated, or only labeling, cleaning, delivery or assembly issues with no use event. 10. U: a device problem is alleged but you cannot tell which kind. - N: no event involving a bed or bed rail is described. ### FLAGS (y or n) - injury: the report states harm to the body or medical care (bruise, skin tear, fracture, laceration, hospital, death). y when the event type is Death. - fall: the person fell out of or off the bed, or to the floor from it. ### SETTING (one letter) - H: a private home, a family member or a home carer is named. - C: a care setting: nursing home, assisted living, hospital, hospice facility, or staff working there. - U: cannot tell. ### FINDING (one letter): what the maker found, as the report states it - D: a fault was found or confirmed, or a part was replaced because it was faulty. - N: no fault found, worked as designed, or the cause was outside the device (wrong rail or mattress for the bed, misuse, modification). - X: not evaluated, not returned, or nothing stated. ### note At most 15 words: the part, what made the choice hard. No quotes longer than 6 words. No personal details. ## Output format CSV with header `report_key,device,category,injury,fall,setting,finding,note`, one row per report in your chunk, nothing else.
