# Codebook: FDA reports on electric positioning chairs (lift chairs and power lift recliners) Written before any report in the set was coded. Mobility Atlas study, October 2026. ## Pool Every FDA MAUDE device report with text filed under product code INO (Chair, Positioning, Electric; 21 CFR 890.3110), received at any date up to the latest openFDA update read on 19 October 2026. The code covers both home lift chairs and clinical positioning chairs, so the first field says which. ## What you record for each report One line per report: `report_key,device,problem,injury,fall,fire,user,finding,note` ### DEVICE (one letter): the kind of product the report is about (text first, then the brand in the header) - L: LIFT CHAIR OR POWER LIFT RECLINER for the home (a recliner or chair whose motor tilts the seat up to help the user stand; Pride, Golden, Med-Lift, Catnapper, La-Z-Boy and similar). - P: CLINICAL POSITIONING CHAIR (hospital or clinic recliner, treatment, dialysis, procedure, blood-draw, stretcher-chair or mobile patient chair). - S: another seat lift for the home (a seat-lift cushion or a lift mechanism added to an existing chair, a powered chair that is not a recliner). - U: an electric positioning chair, but home or clinical cannot be told. - O: another device (bed, wheelchair, patient lift, anything else). - N: no event (blank, administrative, duplicate notice with no event). ### PROBLEM (one letter): what the report says went wrong FIRST (code the allegation even if the maker found no fault). If several apply, use the first in this list. - T: TRAPPED, PINCHED OR CAUGHT: a person, a limb, a child or a pet caught, crushed or pinned by the moving mechanism, footrest or under the chair. - E: ELECTRICAL: hand control or remote, power supply or transformer, cord or cable, wiring, battery backup, motor control board; smoke, fire, sparks, melting, burning smell or shock. - M: MOTOR OR LIFT MECHANISM: actuator or motor failed, chair stuck up, down or reclined, would not lift or lower, moved by itself, lifted or lowered too fast, unexpected movement. - S: STRUCTURE: frame, weld, bolt, arm, back, seat, footrest or linkage broke, bent, cracked or came loose. - F: FALL OR TIP WITHOUT A FAULT: the user fell or slid while getting in or out, or the chair tipped, and no part is said to have failed. - U: USE EVENT WITHOUT A FAULT that is not a fall (skin, pressure, positioning, burns from a heat function with no electrical fault). - Q: a device problem is alleged but no part or behaviour is named. - X: an event is described but nothing failed and no fall or use event fits above. - N: no event. ### FLAGS (y or n) - injury: harm to the body or medical care is stated (bruise, cut, fracture, burn, stitches, hospital, death; pain alone counts). y when the event type is Death. - fall: a person fell or went to the ground (from the chair, out of it, or with it). - fire: the report states fire, smoke, sparks, melting or charring of the chair or its parts. - user: the report says the user or a third party caused it (misuse, a child or pet using the control, wrong installation, a non-approved part or extension cord, a modification), as the cause or the maker's conclusion. ### FINDING (one letter): what the maker found - D: a fault was found or confirmed, or a part was replaced because it was faulty. - N: no fault found, worked as designed, or the cause was outside the device. - X: not evaluated, not returned, or nothing stated. ### note At most 15 words: device, problem, what made it hard. No quotes longer than 6 words. No personal details. ## Output format CSV with header `report_key,device,problem,injury,fall,fire,user,finding,note`, one row per report, nothing else.
