# Codebook: FDA reports on manual wheelchairs (product code IOR) Written before any report in the set was coded. Mobility Atlas study, October 2026. ## Pool (how the reports were found) Every FDA MAUDE device report filed under product code IOR ("Wheelchair, Mechanical") and received by the FDA from 1 January 2023 to the last openFDA update read on 10 October 2026 that carries narrative text, as downloaded from api.fda.gov on 10 October 2026. For 2023 to 2025 every report was downloaded (441, all with text). For 2026, 3,060 reports were received and 1,019 carry text; the 2,041 without any text cannot be read and are counted but not coded. Pool: 1,460 reports. The narrative is the "Description of Event or Problem" plus every other text section of the report (manufacturer narratives included). ## What you record for each report One line per report: `report_key,chair,category,injury,fall,arrival,retro,note` ### CHAIR (one letter): what kind of manual wheelchair, ONLY as the report states it - T: a transport chair (pushed by a companion, small rear wheels; the report says "transport") - A: an active, ultralight or rigid chair, or a sports chair (the report says so, or names a model sold as one: Quickie 2/QXi/Nitrum/Xenon/Argon/Krypton, TiLite, Ki Mobility Rogue/Catalyst/Little Wave, Zippie, Panthera, Küschall, Motion Composites, Invacare Top End) - S: a standard, lightweight folding, hemi-height, extra-wide or bariatric chair (the report says so, or names such a model: Medline Guardian/Excel/K1/K2/K3/K4, Drive Silver Sport, Invacare Tracer, Everest & Jennings) - P: a tilt-in-space, recliner or positioning chair (Quickie Iris, Ki Focus CR, recliner) - U: not stated or cannot tell Do not guess from anything else. ### RELEVANCE - N: no event or problem involving the wheelchair is described (blank, administrative only, or about another product). Record N in the category column, chair U, flags n. ### CATEGORY (one letter): what the report says went wrong. Precedence: the FIRST letter that applies wins. 1. L: WHEEL LOCKS or brakes: the wheel locks (parking brakes), attendant brakes or hand brakes did not hold, would not engage, came loose, slipped or broke; the chair rolled because a lock failed. 2. F: FRAME or folding mechanism: the frame, cross brace or crossbar, seat rail, side frame, weld, folding or unfolding mechanism, or back post/back cane cracked, bent, broke, separated, or would not open or lock. 3. W: WHEELS: a rear wheel, quick-release axle, tire, rim, spoke, handrim, caster, caster fork, caster stem or bearing came off, cracked, broke, went flat or locked up. 4. R: other ATTACHED parts: footrest, leg rest, footplate, armrest, push handle, anti-tipper, seat or back upholstery, seat belt, hardware such as a screw, bolt, nut or cap came loose, broke or detached; sharp edges or exposed bolts. 5. T: TIP-over, fall or loss of stability and the report does NOT name a failed part from 1-4. 6. X: the report describes an event but states NO wheelchair problem (user or caregiver action, medical event, hit by something, the manufacturer found no fault) and none of 1-5 is alleged. 7. U: a problem is alleged but you cannot tell what failed. If the reporter alleges a failure and the manufacturer says it found none, code the ALLEGED failure and note "maker found no fault". ### FLAGS (y or n) - injury: the report states that a person was injured, needed medical care, or died. "No injury reported" = n. - fall: the report states that the user fell or slid out, or the chair tipped over, whatever the cause. - arrival: the problem was found when the chair arrived or was first opened, unfolded or assembled ("defective on arrival", "out of the box", "brand new", "missing parts", "first use"). - retro: the report says it was filed as part of a retrospective review of historical complaints ("retrospective review", "retrospective review activities", "historical data"). ### note At most 15 words: the part named and anything that made the choice hard. No quotes longer than 6 words. Never include personal details. ## Output format CSV with header `report_key,chair,category,injury,fall,arrival,retro,note`, one row per report in your chunk, nothing else. Every report in the chunk gets exactly one row.
