# Codebook: FDA reports on mobility scooters (product code INI, plus ITI reports that name a scooter) Written before any report in the set was coded. Mobility Atlas study, October 2026. ## Pool Every FDA MAUDE device report under product code INI ("Vehicle, Motorized 3-Wheeled", which in practice holds 3- and 4-wheel mobility scooters) received from 1 January 2016 to the latest openFDA update read on 10 October 2026 that carries narrative text (416 of 418), plus the 4 reports under product code ITI ("Wheelchair, Powered") received from 1 January 2024 whose text or product name says "scooter". Pool: 420 reports. The narrative is the event description plus every other text section (manufacturer narratives included). ## What you record for each report One line per report: `report_key,kind,category,injury,fall,vehicle,retro,note` ### KIND (one letter): what device, ONLY as the report states it - S: a mobility scooter (the report says scooter, or names a scooter model: Pride Go-Go, Victory, Revo, Baja, Maxima, Celebrity, Raptor, Zero Turn; Golden Buzzaround, Companion, LiteRider; Drive Scout, Spitfire, Phoenix, ZooMe; EV Rider; Afikim/Afiscooter; Hoveround scooters; Merits scooters) - F: a folding or travel scooter the report calls folding or travel - C: a power wheelchair (joystick, the report calls it a wheelchair or power chair) - U: not stated or cannot tell Do not guess from the product code alone. ### RELEVANCE - N: no event or problem involving the device is described (blank, administrative, about another product). Record N in the category column, kind U, flags n. ### CATEGORY (one letter): what the report says went wrong. Precedence: the FIRST letter that applies wins. 1. F: FIRE, smoke, melting, sparks, burning smell, battery or charger overheating or bursting. 2. M: UNCONTROLLED MOVEMENT: the scooter moved, accelerated, kept going, reversed or would not stop when the user did not intend it (throttle lever stuck, "took off", "would not stop"). 3. B: BRAKES or rolling: brakes or freewheel did not hold, the scooter rolled on a slope or ramp, or would not stop because braking failed. 4. S: STEERING: the tiller, handlebars, tiller lock or steering linkage broke, came loose, locked or would not steer. 5. W: WHEELS: a wheel, tire, axle, wheel hub, caster or anti-tip wheel came off, broke, went flat or locked. 6. E: ELECTRICAL or power: power loss, the scooter stopped or would not move, error codes, controller, wiring (no fire). 7. R: other PART broke or came loose: seat, seat post or swivel, frame, weld, floor, basket, armrest, mirror, lights, hardware. 8. T: TIP-over or fall from the scooter and the report names NO failed part from 1-7 (includes curbs, ramps, slopes, turning, transfers). 9. X: an event with NO scooter problem stated (struck by a vehicle with no fault named, a medical event, user action, the maker found no fault) and none of 1-8 is alleged. 10. U: a problem is alleged but you cannot tell what failed. If the reporter alleges a failure and the manufacturer says it found none, code the ALLEGED failure and write "maker found no fault" in the note. ### FLAGS (y or n) - injury: the report states harm to the body or medical care (injury, wound, fracture, bruise, burn, pain, hospital, ER, died). A stated impact alone is n. - fall: the user fell or was thrown from the scooter, or the scooter tipped over. - vehicle: the user was struck by a car, truck, bus or other motor vehicle. - retro: the report says it was filed as part of a retrospective or historical review of complaints. ### note At most 15 words: the part or place named and anything that made the choice hard. No quotes longer than 6 words. No personal details. ## Output format CSV with header `report_key,kind,category,injury,fall,vehicle,retro,note`, one row per report in your chunk, nothing else. Every report gets exactly one row.
