# Codebook: FDA reports on patient lifts and slings (product codes FSA and FNG) Written before any report in the set was coded. Mobility Atlas study, October 2026. ## Pool Every FDA MAUDE device report filed under product code FSA ("Lift, Patient, Non-AC-Powered") or FNG ("Lift, Patient, AC-Powered") received from 1 January 2025 to the latest openFDA update read on 11 October 2026 (data to the end of August 2026): 1,909 reports, of which 957 carry text. All 957 are in the pool. The codes also cover bath and shower lifts, chairs and trolleys and sit-to-stand aids; the DEVICE field records which. ## What you record for each report One line per report: `report_key,device,category,injury,fall,setting,finding,note` ### DEVICE (one letter), from the report text; the brand line may be used when the text does not say - F: FLOOR or mobile lift with a boom and a sling (Hoyer-type, Maxi Move, Maxi 500, Viking, Golvo, most Medline patient lifts). - C: CEILING or overhead track lift (Maxi Sky, Likorall, track or rail lift). - S: SIT-TO-STAND or stand-aid lift (Sara, stand assist, standing lift). - B: BATH or shower lift, bath chair, shower trolley or hygiene chair (Alenti, Carevo, bath lift). - O: something else, or the report does not say what kind of lift. ### CATEGORY (one letter): what the report says went wrong. Precedence: the FIRST letter that applies wins. 1. G: SLING: the sling tore, frayed or wore through, or a strap, loop, clip, buckle or leg piece broke, came loose or detached, or the wrong or worn sling was used, or loops were not attached properly. 2. H: HANGER: the spreader bar, hanger bar, hook, latch or the attachment between the lift and the sling broke, opened or detached. 3. T: TIP: the lift tipped over, nearly tipped, or its base, legs or wheels were unstable or gave way. 4. D: DROP: the lift lowered, dropped or descended on its own, suddenly or too fast, with or without a person in it. 5. M: STRUCTURE: another part of the lift broke, cracked, bent or came loose (mast, boom, column, track, rail, motor housing, brake, caster, seat or support of a bath or stand-aid device). 6. E: ELECTRICAL OR CONTROLS: the lift would not raise, lower or move, stopped part way, the hand control, control box, actuator, battery or charger failed, or the emergency stop or emergency lowering did not work or was missing. 7. P: CONTACT INJURY with no failure from 1-6: a person was pinched, struck by the spreader bar or boom, scraped or crushed during use. 8. F: FALL or slip out of the sling or device with no failure from 1-7 named (including a person sliding out of a correctly attached sling). 9. X: no device problem stated, or only cleaning, labelling, service or delivery issues. 10. U: a device problem is alleged but you cannot tell which kind. - N: no event involving a patient lift or bath lift device is described. ### FLAGS (y or n) - injury: the report states harm to the body or medical care for the person lifted or a caregiver (wound, fracture, bruise, laceration, staples, stitches, hospital, death). An impact alone is n. y when the event type is Death. - fall: the person being lifted fell, slid out, or was dropped to the floor or another surface (a near fall caught by staff is n). ### SETTING (one letter) - C: a care setting: hospital, nursing home, care home, rehabilitation, staff such as nurses, CNAs or caregivers named as working there. - H: a private home, family member or home carer. - U: cannot tell. ### FINDING (one letter): what the maker or service found, as the report states it - D: a fault was found or confirmed, or a part was replaced because it was faulty. - N: no fault found, could not duplicate, worked as designed, or the cause was outside the device (use error, wrong sling, not inspected, modification). - X: not evaluated, not returned, or nothing stated. ### note At most 15 words: the part, what made the choice hard. No quotes longer than 6 words. No personal details. ## Output format CSV with header `report_key,device,category,injury,fall,setting,finding,note`, one row per report in your chunk, nothing else.
