# Codebook: FDA reports about batteries, chargers and charging on power wheelchairs and scooters Written before any report in the set was coded. Mobility Atlas study, October 2026. ## Pool FDA MAUDE device reports with narrative text that mention a battery, a charger or charging, fire, smoke, melting or sparks (word search on the report text), from three sets: (a) product code ITI ("Wheelchair, Powered") received 1 January 2019 to 31 December 2023, found by openFDA text search for battery, batteries, charger, charging or fire (88 reports); (b) product code ITI received from 1 January 2024 to the latest update read on 10 October 2026 (the 1,296-report Mobility Atlas set) whose text matches BATTER, CHARG, FIRE, SMOKE, MELT or SPARK (48 reports); (c) product code INI (mobility scooters) received from 1 January 2016 to the same date, with the same text match (87 reports). Pool: 223 reports. Many mention a battery only in passing; the codebook sorts those out. ## What you record for each report One line per report: `report_key,device,category,injury,charging,lithium,note` ### DEVICE (one letter), ONLY as the report states it - C: power wheelchair; S: mobility scooter; A: a power add-on on a manual chair (SmartDrive, e-motion, Firefly and similar); U: cannot tell. ### CATEGORY (one letter): what the report says the battery or charging problem was. Precedence: the FIRST letter that applies wins. 1. F: FIRE, flames, smoke, melting, explosion or burst, on the device, its battery, its charger, its wiring or the outlet. 2. H: OVERHEATING without fire: a battery, charger, cable or connector got hot, or a person was burned by a hot part. 3. L: BATTERY DAMAGE: leaking acid, swelling, cracked case, corroded terminals, or a person harmed by battery acid or by handling a battery. 4. C: CHARGER or CHARGING fault without heat or fire: the charger failed, would not charge, wrong charger used, charging port or cord broken. 5. W: WIRING or CONNECTOR fault in the battery or charging circuit without heat or fire: loose, cut, pinched or disconnected battery harness, fuse, terminal. 6. D: POWER LOSS blamed on the battery: dead or drained battery, short range, battery died while driving, the device stopped and the report names the battery. 7. O: the battery or charging is mentioned but the problem the report describes is something else (for example "the battery was charged" in a report about a tip-over). 8. U: a battery or charging problem is alleged but you cannot tell which kind. - N: no event or problem involving the device is described. ### FLAGS (y or n) - injury: the report states harm to the body or medical care (burn, smoke inhalation, injury, hospital, death). Damage to property alone is n. - charging: the event happened while the device or battery was charging or plugged in. - lithium: the report states the battery was lithium (lithium-ion, LiFePO4, Li-ion). ### note At most 15 words: the part named (battery, charger, harness, outlet), the place (home, outside), and anything that made the choice hard. No quotes longer than 6 words. No personal details. ## Output format CSV with header `report_key,device,category,injury,charging,lithium,note`, one row per report in your chunk, nothing else. Every report gets exactly one row.
