# Codebook: FDA reports naming a drive, brake or control part on powered wheelchairs (product code ITI) Written before any report in the set was coded. Mobility Atlas study, October 2026. ## Pool FDA MAUDE device reports under product code ITI ("Wheelchair, Powered") received from 1 January 2019 to the latest openFDA update read on 10 and 11 October 2026, with narrative text, that do NOT name a power add-on (SmartDrive, e-motion, e-fix, SMOOV, Firefly, Twion, power assist, PushTracker, Switch-It, SpeedControl), and whose text matches MOTOR, BRAKE, GEARBOX, CONTROLLER, POWER MODULE, MODULE, JOYSTICK, ERROR CODE, FAULT CODE, WON'T / WOULD NOT / NOT MOVE, STOPPED, SHUT OFF / SHUT DOWN, FREEWHEEL, CLUTCH, ENCODER or BEEP (413 reports). Many match a word only in passing; the codebook sorts those out. ## What you record for each report One line per report: `report_key,device,category,part,injury,fall,stranded,finding,note` ### DEVICE (one letter), ONLY as the report states it - C: power wheelchair; S: mobility scooter; A: power add-on on a manual chair; O: something else or cannot tell. ### CATEGORY (one letter): what the report says went wrong. Precedence: the FIRST letter that applies wins. 1. M: unintended MOTION: the chair moved, sped up, veered, spun or kept going when the user did not command it, or would not stop when commanded. 2. B: BRAKES or holding: the chair rolled away or would not hold on a slope, brakes failed, a freewheel lever was left out or slipped. 3. F: FIRE, smoke, melting, burning smell, sparks or overheating from any part. 4. E: STOPPED or no drive: the chair stopped suddenly, cut out, would not move, would not turn on, showed an error or beeped and would not drive, drove only slowly or only one way, controls or display dead. 5. P: POWERED SEATING: tilt, recline, elevator, standing or leg-rest actuators moved unexpectedly, failed or trapped a body part. 6. S: STRUCTURAL: a part broke, cracked, came loose or fell off (frame, seat, wheel, caster, motor mount, joystick mount), with none of 1-5. 7. T: TIP or fall with no device fault from 1-6 named. 8. X: event with no device problem stated, or the problem is only service, delivery or billing. 9. U: a device problem is alleged but you cannot tell which kind. - N: no event involving a powered mobility device is described. Leave the other fields n or X. ### PART (one letter): the part the report names as faulty, whether alleged or found; the FIRST that applies - J: joystick, hand control or other input device (remote, display, switch, head array, sip-and-puff, attendant control) or its cable. - K: controller, power module, electronics or wiring other than the battery circuit. - G: drive motor or gearbox (including motor brush, encoder, motor cable or connector). - B: brake (electromagnetic or parking) or freewheel / release lever. - W: wheel, tyre, caster or fork. - A: seat actuator or powered-seating part. - T: battery, charger, fuse or battery harness. - O: another named part. - N: no part named. ### FLAGS (y or n) - injury: the report states harm to the body or medical care. An impact alone is n. - fall: the user fell or slid out of the chair, or the chair tipped. - stranded: the user was left stuck: could not move, needed someone (family, staff, fire department, police) to move them, or was trapped in a position. ### FINDING (one letter): what the maker or service found, as the report states it - D: a fault was found or confirmed, or a part was replaced because it was faulty. - N: no fault found, could not duplicate, worked as designed, or cause outside the device (misuse, environment, modification). - X: not evaluated, not returned, or nothing stated. ### note At most 15 words: the part, the place, what made the choice hard. No quotes longer than 6 words. No personal details. ## Output format CSV with header `report_key,device,category,part,injury,fall,stranded,finding,note`, one row per report in your chunk, nothing else.
