# Codebook: FDA reports on stairlifts and wheelchair platform lifts (product codes PCD and PCE) Written before any report in the set was coded. Mobility Atlas study, October 2026. ## Pool Every FDA MAUDE device report filed under product code PCD ("Powered Stairway Chair Lift", 21 CFR 890.5150) or PCE ("Permanently Mounted Wheelchair Platform Lift", 21 CFR 890.3930) in openFDA, read on 15 October 2026: 234 reports, of which 231 carry text. All 231 are in the pool. ## What you record for each report One line per report: `report_key,device,category,injury,fall,stranded,finding,note` ### DEVICE (one letter), from the report text; the brand line may be used when the text does not say - S: a STAIRLIFT with a seat that travels on a rail (Acorn, Handicare, Bruno, Stannah, Harmar stair chair). - P: a PLATFORM lift for a wheelchair, vertical or inclined (porch lift, vertical platform lift, inclined platform). - O: something else, or the report does not say. ### CATEGORY (one letter): what the report says went wrong. Precedence: the FIRST letter that applies wins. 1. H: FIRE, smoke, sparks, melting, burning smell or overheating. 2. M: UNEXPECTED MOVEMENT: the lift moved on its own, did not stop when released or at the end of the rail, overran, ran down the rail, or moved while the user was getting on or off. 3. C: ENTRAPMENT OR CRUSH: a body part, clothing or object was caught, pinched or crushed by the moving lift, footrest, carriage or platform. 4. R: RAIL OR DRIVE: the rail, gear rack, carriage, drive, rollers or mounting broke, came loose or failed. 5. B: SEAT OR SAFETY PART: the seat, swivel, armrest, footrest, seatbelt, sensor, gate, ramp or barrier failed, would not lock or broke. 6. E: STOPPED OR NO DRIVE: the lift stopped part way, would not move, error or beeping, batteries, charging, remote or switch failed. 7. T: FALL or tip with no failure from 1-6 named (getting on or off, leaning, belt not worn, user lost balance). 8. X: no device problem stated, or only installation, service, sales or billing issues. 9. U: a device problem is alleged but you cannot tell which kind. - N: no event involving a stairlift or platform lift is described. ### FLAGS (y or n) - injury: the report states harm to the body or medical care (bruise, fracture, laceration, hospital, death). An impact alone is n. y when the event type is Death. - fall: the user fell from the lift, on the stairs, or from the platform. - stranded: the user was left stuck on the lift or the stairs, or needed someone to free or move them. ### FINDING (one letter): what the maker or service found, as the report states it - D: a fault was found or confirmed, or a part was replaced because it was faulty. - N: no fault found, could not duplicate, worked as designed, or the cause was outside the device (use error, belt not worn, installation by others, modification). - X: not evaluated, not inspected, or nothing stated. ### note At most 15 words: the part, what made the choice hard. No quotes longer than 6 words. No personal details. ## Output format CSV with header `report_key,device,category,injury,fall,stranded,finding,note`, one row per report in your chunk, nothing else.
