# Codebook: FDA reports on tilt-in-space and reclining wheelchairs Written before any report in the set was coded. Mobility Atlas study, October 2026. ## Pool FDA MAUDE device reports filed under product code IOR (Wheelchair, Mechanical) or ITI (Wheelchair, Powered) whose text contains "tilt in space", "tilt-in-space", "power tilt", "tilt system", "tilt function", "tilt actuator", "tilt module", "tilt mechanism", "space tilt", "recline", "reclining" or "power recline", read in openFDA on 22 October 2026: every such report received before 2011 or from 2016 on, and a systematic sample of 200 of the 1,118 received from 2011 to 2015. ## What you record for each report One line per report: `report_key,device,seating,problem,injury,fall,user,finding,note` ### DEVICE (one letter): the chair the report is about - P: a POWER WHEELCHAIR. - M: a MANUAL WHEELCHAIR (including manual tilt-in-space and reclining chairs and attendant-propelled chairs). - O: another device. - N: no event. ### SEATING (y or n) - y: the event involves the chair's tilt, recline or seat-elevation function, or the parts that do it (actuator, gas spring, release cable or handle, tilt or recline frame, seating module, seating switch). - n: tilt or recline is only mentioned (for example the chair has it), and the event is about something else. ### PROBLEM (one letter): what the report says went wrong FIRST (code the allegation even if the maker found no fault). If several apply, use the first in this list. - P: PINCHED, CAUGHT OR TRAPPED: a body part caught, pinched or crushed by the moving seat, back, elevator or leg rest, or the user trapped in a position. - A: TILT, RECLINE OR ELEVATE MECHANISM: actuator, motor, gas spring, cable, release handle or lock failed; seat stuck tilted, reclined or raised; would not tilt, recline or return; moved by itself, too fast or too far; seat or back dropped or flipped back. - C: SEATING CONTROL OR ELECTRICAL: seating switch, module or wiring; smoke, fire, sparks or melting in the seating system. - S: SEAT OR BACK STRUCTURE: seat frame, back canes, recline or tilt frame, bracket, bolt or weld broke, cracked, bent, loosened or wobbled. - T: TIP OR FALL WITHOUT A FAULT ABOVE: the chair tipped over or the user fell or slid out, including while tilted or reclined. - G: ANOTHER PART OF THE CHAIR: drive, motor, brakes, wheels, casters, joystick for driving, battery, leg rests, armrests, anything not in the seating system. - U: BODY EVENT WITHOUT A FAULT that is not a fall: skin or pressure injury, positioning, breathing, pain, with no part said to have failed. - Q: a device problem is alleged but no part or behaviour is named. - X: an event is described but nothing failed and nothing above fits. - N: no event. ### FLAGS (y or n) - injury: harm to the body or medical care is stated (bruise, cut, fracture, burn, stitches, hospital, death; pain alone counts). y when the event type is Death. - fall: the user fell or went to the ground, or the chair tipped over with the user. - user: the report says the user or a third party caused it (misuse, wrong setup, a modification, an unapproved part, use against the instructions), as the cause or the maker's conclusion. ### FINDING (one letter): what the maker found - D: a fault was found or confirmed, or a part was replaced because it was faulty. - N: no fault found, worked as designed, or the cause was outside the device. - X: not evaluated, not returned, or nothing stated. ### note At most 15 words: chair, problem, what made it hard. No quotes longer than 6 words. No personal details. ## Output format CSV with header `report_key,device,seating,problem,injury,fall,user,finding,note`, one row per report, nothing else.
