Sling upholstery sags with time, and many users replace it with a shaped back. Rigid backrests such as the JAY J3 Back, Matrx E2 Back and Spex Vigour Back Support are padded shells that mount on the back canes with adjustable hardware and come in several heights and contour depths. Dynamic or flexible backrests give a little as the user moves. Lateral trunk supports are pads at the sides of the ribcage that limit leaning, and headrests support the head during tilt, rest or transport. Selection and fitting are done with a clinician or seating specialist.

A rigid shell keeps its shape, so the pelvis and spine get consistent support and less energy is lost in a sagging sling when pushing. Angle, height and depth can usually be adjusted.
They are padded brackets fixed to the backrest on one or both sides of the trunk. They help a person who leans or cannot hold the midline stay upright, and many swing away for transfers.
